As part of our commitment to providing the highest standard of care, Sulis Hospital Bath collects information from patients before and after certain key treatments. This helps us better understand how treatment has impacted your health and quality of life, enabling us to continually improve outcomes for both current and future patients.
If you are scheduled for a qualifying procedure, you may be contacted via email and/or SMS by our trusted partner, Amplitude Clinical Outcomes, at key stages throughout your care journey. This could continue for several years after you have completed your treatment to help establish long-term outcomes.
Please do not be concerned if you do not receive any correspondence from Amplitude. You do not need to take any action if you do not receive an invitation.
We will appreciate your cooperation with this process. Your participation plays an important role in helping us continually improve the care we provide. Participation is completely voluntary and will not affect your treatment or care.
All information you provide will be handled securely and in accordance with the Data Protection Act and our privacy policy.
If you experience any technical issues or require additional support when using the Amplitude Patient Portal, you can contact the Amplitude Customer Support team: customer.support@amplitude-clincal.com / 0333 014 63 63 (UK) | Int: +44 (0)1905 932 575
If you have any questions or concerns about the questionnaires, or any questions about your treatment, please contact us directly.
As Sulis Hospital, your health and well-being are at the heart of everything we do. While medical scans and test results are important, they only tell part of the story. The most vital piece of information is how you actually feel.
To make sure we are listening to you every step of the way, we partner with a secure digital system called Amplitude. Before and after your treatment, you will receive simple online questionnaires asking about your pain levels, mobility and how you are managing daily life.
PROMs are a way of recording information about your health, wellbeing and quality of life directly from you, the patient. They allow us to understand and compare how you are feeling and functioning before and after a clinical intervention.
Your experience as a patient at Sulis Hospital Bath and the outcome or your treatment is important to us. The information you provide helps us to understand the outcomes of treatment, supports us in identifying the best treatments for future patients and may also contribute to formal research trials.
By inputting your progress in our system, you are not only helping us by giving us the ability to report on all our patients, but you are also helping yourself. Clinical outcomes are a way for you, your surgeon and other healthcare professionals to monitor your symptoms and the quality of our clinical interventions.
Whilst your cooperation is not compulsory, your input is important to this process. Sulis Hospital is using the Amplitude System to record and report on your progress as well as the progress of hospital patients as a collective.
